When left to individuals, the value of the gift may decrease by up to 70%, but if used to make a gift to the Hospital, the funds are transferred tax-free and we are able to realize the full value of the gift.

Contingent Bequest Language

Retirement Plan Beneficiary Language

Making a charitable gift from your retirement plan is easy and should not cost you any attorney fees. Simply request a change of beneficiary form from your plan administrator. When you are done, please return the form to your plan administrator and notify Springfield Hospital.
