Requesting Your Medical Record

If you would like to obtain a copy of your Medical Records from Springfield Hospital, The Windham Center for Psychiatric Care, Springfield Area Adult Day Service, Physical Therapy, and/or Springfield Hospital Specialty Practices (General Surgery, Podiatry, Connecticut Valley Orthopaedics, Gynecology, Urology, Springfield ENT, Pain Clinic, Cardiology, Oncology and Infusion clinic), please call:

802-885-7304 after hours, please leave a message and we will return your call to assist you in getting your records.

You will need to submit a completed and signed Springfield Hospital/Specialty Practices medical records release form.

Medical Records related potentially to RHC (Reproductive Health Care) require the requester to complete an additional attestation that the records related to RHC are not being sought for a prohibited purpose.  The additional attestation form can be downloaded, completed and signed, and is required in addition to the standard release form.  

If you wish to obtain records from North Star FQHC (formerly Springfield Medical Care Systems), please contact the North Star Medical Records department by emailing to: records@northstarfqhc.org and they will be happy to assist you in getting your records from North Star.
