Nutrition Services at Springfield Hospital include provision of therapeutic diets based on patients’ individual nutrition needs, development of nutrition care plans for patients at nutritional risk, as well as nutrition and diabetes education for patients admitted to the hospital Inpatient Care Unit. The hospital’s clinical dietitian also provides nutrition services to individuals attending Springfield Adult Day Service and for patients admitted at The Windham Center for Psychiatric Care.
Individuals who need nutrition and diabetes counseling on an outpatient basis are seen through North Star Health’s Community Health Team and should request a referral from their primary care provider or contact the Community Health Team at 802-886-8998 to schedule an out-patient appointment with a dietitian. North Star Health also has free health and wellness classes available to all in the community.
